Session will highlight navigation models of delivery so that navigators can advocate for patients with cancer by utilizing evidence-based shared decision- making tools, best practices in coordinating care, and assessment strategies in addressing barriers to care. This forum will provide the navigator exposure with other navigation and survivorship care colleagues into evidence – based navigation research studies, quality improvement projects, and best practices utilizing pre- and post-data elements to enhance their navigation practice.
