Description

This session will discuss the complex practice of multi-site navigation. It will describe the key role of patient/clinical navigators in assisting patients of various tumor sites with proper treatment modalities and identifying barriers to care. Navigating multiple tumor sites can be very challenging. These challenges will be discussed and tools and resources will be presented. Navigation metrics that can be used by multi-site navigators will also be reviewed.

Learning Objectives:
  • Discuss the role of the clinical/patient navigator in supporting patients throughout the cancer trajectory
  • Review tools and resources available
  • Identify barriers and challenges that the clinical/patient navigator has in multi-site navigation
  • Discuss navigation metrics that can be used in multi-site navigation.
